The eighth National Congress of Chronic Patient Health Care was held in Madrid on 7 and 8 April 2016, continuing the debate on the models of care for chronicity initiated in previous congresses.
Its central theme was "Advanced Results" achieved after nearly 10 years of development of different strategies addressing chronicity and four years since the adoption of the strategy at national level. The central issues such as progress in autonomy and self-care, integrated care processes, socio-health coordination, innovation, research, etc. were organized through discussion and round tables, conferences, workshops, communication sessions on practical experiences of success and projects that are contributing positively to changing the delivery of care for patients.
In addition, a space was created where citizens and health professionals and social services could reflect together to find new ways and new solutions for chronic patients and their families, including their views and opinion in discussions, debates and workshops held as part of the third Active Patient National Conference.
Readi for Health project was represented by the partner from Murcia (FFIS) as a speaker in the practical workshop "If you do not listen to me, I can not help you". How to improve the effectiveness of drugs in chronic polypharmacy patients? attended by about 35 participants. FFIS also participated in the panel discussion "Moving forward in implementing strategies" where about 150 professionals and patients expressed their views about the barriers to implementing the reorganization of the health system to help guide the care of chronic patients. These topics of discussion are directly linked to research conducted by the Consortium in work package 3.
